MDS Coordinator Requirements:
- Applicant must possess strong communication, clinical, and organizational skills.
- MDS experience preferred.
- Current/active RN/LPN licensure required.
MDS Coordinator Job Duties:
- Transcribe and take appropriate action on physician’s orders as directed including but not limited to Care Plan changes.
- Assist with performing all admitting and discharge assessments and complete all other necessary paperwork.
- Help monitor all aspects of patient care.
- Initiate requests for consultation or referral. Respond to requests from the resident, physician, or nursing staff.
- Cooperate with other resident services when coordinating nursing services to ensure that the resident’s total regimen of care is maintained.
- Coordinate and prepare MDS assessments for all residents.
- Oversee the transmission of MDS resident assessments.
- Prepare, monitor, and update assessments, Care Area Assessments, and Care Plans, as necessary.
- Participate in Care Plan meetings with residents and family members.
- Be aware of residents’ exit to and reentry from hospitals.
