What are the responsibilities and job description for the Residential Case Manager position at Heartly House, Inc. of Frederick County, Maryland?
Heartly House supports survivors of intimate partner violence, sexual assault, human trafficking and child abuse by providing safety, emergency shelter and supportive services. The Residential Services Case Manager is a full-time, 35 hours / week benefited, non-exempt position that reports directly to the Residential Services Director.
AREAS OF RESPONSIBILITIES
· Provides case management and service coordination to clients residing in Heartly House shelter and/or participating in rental assistance programs
· Provides shelter coverage and direct client services during daytime, evening, and weekend shifts.
· Conducts client screening and assessment for Shelter program. Teams with Residential Services Director or Residential Programs Manager on eligibility as needed.
· Completes all paperwork related to shelter intake/admittance
· Regularly and frequently monitors and assesses clients in achieving their goals. Residential clients are entitled to a minimum of a one-hour case management appointment once a week.
· Advocates on behalf of clients to ensure access to service resources outside of Heartly House
· Enforces shelter policies and procedures, assists with room turnover as directed, assists with House Meetings and Room checks as directed
· Provides crisis intervention to clients residing in shelter
· Mediates conflicts between clients residing in shelter
· Assists with inventory and management of food, shelter supplies, and donations as directed
· Maintains accurate case and financial records
· Assists with preparation of grant applications, statistical, and narrative program reports for residential programs as directed.
· Collaborates and assists with coordinating client needs across Heartly House departments
· Maintains client confidentiality at all times
· Maintains shelter security at all times
· Meets with Residential Services Director regularly for supervision as directed
· Attends staff and clinical meetings
· Adheres to Heartly House philosophy, policies and procedures
· Other duties as assigned by Residential Services Director
EDUCATION AND EXPERIENCE
· BA/BS in Human Services or related field. Experience in shelter settings preferred. Related practical experience may be substituted for a portion of education
· Experience in case management/service coordination, shelter settings, and with clients experiencing trauma and homelessness
· Demonstrated proficiency with interpersonal communications is essential
· Strong computer literacy including Microsoft Office Suite is required and experience using data collection software is preferred
· Fluency in Spanish preferred
· Maturity, reliability, dependability
· Ability to assess situations, work independently, show initiative and flexibility in meeting client needs
· Ability to motivate others
· Working knowledge of Frederick County human service agencies preferred
· Knowledge of trauma-informed services and working within an empowerment model preferred.
· Must successfully complete Heartly House training program within introductory employment period
