The Intake Clinician will provide clinical intakes to our foster care population and their families. The intake clinician will also carry a small caseload of clients.
Responsibilities: 29I; Intake Coordinator
- Complete assessments, screenings, and treatment evaluations for youth entering into the facilities care or provide updated assessments for youth already in care. Complete client intakes in office and/or virtual
- After completion of initial assessments, complete referrals when appropriate for services such as outpatient mental health (external or internal), comprehensive psychological exams, educational services, medication management, or other specialty services
- Work in close collaboration with Care Coordination, Case Planning, & other Admirative teams to assure families attend scheduled appointments
- Attend weekly supervision with Clinical Supervisor
- Attend team meetings with the 29I interdisciplinary team to address monthly attendance rates and practice updates
- Follow-up with the foster care clinical and administrative team to ensure all clients in care complete required evaluations and ensure follow up on referrals
- Any other duties as assigned
- Maintain a small caseload of clients
- Provide clinical updates when necessary and appropriate for the case planning team
- Work in close collaboration with Case Planning & Foster Parents to assure families attend scheduled sessions
- Create and continuously update treatment plans for each client within your caseload
- Properly document and complete clinical notes for each attended session
- Provide monthly summaries regarding each client’s progress/attendance via Connections
- Attend weekly group supervision meetings
- Complete mandatory trainings related to job performance and share clinical knowledge with others to enhance the team's
- clinical perspective
- Any other duties as assigned.
- Master's degree in social work
- Minimum master’s degree with a minimum of 2 years’ experience in children's mental health, addiction, and/or foster care/child welfare/juvenile justice.
- FULLY licensed mental health counselors in NY State only (LMHC).
- Be able to complete Mandated Reporting self-directed online training
- Have experience working with children and young adults
