Position Summary:
As a Job Coash you will support HeartShare Brooklyn and Queens Supported Employment Programs.
Primary Responsibilities and Essential Functions of Position:

The Job Coach will use structured training techniques for teaching job performance skills such as sequence, quality and quantity, as well as job related skills such as grooming, socializing with co-workers, accepting supervision or managing one's paycheck. Help with learning the job duties and the work environment, when job duties change, assist with social interactions on the job and provide ongoing assistance through periodic worksite visits.

The Job Coach will ensure success of the placement by providing advocacy: at the work site, the employees' residence or in the community, and with external provider agencies (such as the Social Security Administration, Department of Social Services, MTA Access-a-Ride).

The Job Coach will assist the employee with transportation needs either by travel training or by reinforcing and continually monitoring employees' overall travel and safety skills.

The Job Coach is responsible for monitoring all employment related activity and progress of the employee. To accomplish this, the Job Coach should have direct contact with the employee, employer and Supported Employment program management.

The Job Coach is required to submit daily and monthly reports to the Program Supervisor. The Job Coach is responsible for visiting the employees' jobsite at least two times each month that the individual is employed. These face to face on-site reports should document summary of consumer progress, including identification of any problem areas and the Employers concerns/suggestions about the employees' overall job performance.
The narrative report must also outline and include:-

All instructions that were provided along with a recommendation for ongoing instruction, including an estimate of anticipated degree of continued job coaching.

Other Activities and Responsibilities

Maintain a positive relationship with the employee, employer and their families.

Become familiar with relevant labor laws, the ADA and must provide benefits counseling when needed. Adhere to OPWDD, ACCES-VR, Dept. of Labor, HIPAA and OSHA regulations.

Collaborate, provide feedback and participate in individual Program Planning Team (PPT) meetings as required.

Provide regular feedback and required daily and monthly progress reports to Program Supervisor as requested.

Respond proactively to the emotional, physical, social and cultural needs of each employee that he/she works with.

Ensure that service delivery is performed in a manner that respects the specific needs and preferences of the individuals being supported, reflecting any differences in culture, religion, life experience and other uniqueness that is part of the individual's background.

Report all incidents on the job to program management immediately upon discovery.

Represent HeartShare, in a professional manner that promotes the mission, vision, and values of the agency and reflects well on HeartShare at all times.

Responsible for developing volunteer, paid jobs and meaningful activities for program individuals.

Adheres to all HeartShare policies and procedures. Abides by agency dress code, as well as any dress codes required by a given community site, including business attire and uniforms, where necessary.

Undertake any additional tasks or initiatives as assigned by Program Supervisor.
Qualifications
- High School Diploma/GED.
- Experience working with individuals with intellectual and developmental disabilities.
- Strong written and verbal communication skills
- Must be willing to travel throughout NYC via public transportation.
