Description

The Case Planner enhances the quality of life of Supportive Independent Living Program (SILP) residents and their families by providing and/or arranging for the provision of social and related services based on service plans and needs; prepares youngsters for independent living, family reunification, or continues care; Maintains compliance with Agency and SILP standards, and government requirements.

Job Responsibilities:

  • Manages a caseload of 12 to 15 residents, along with all aftercare clients, as defined by NYS OCFS and assigned by SILP.
  • Develop individual service plans, using SILP and HSVS guidelines and governmental objectives.
  • Assess tenant needs through interviews and connect them with appropriate resources.
  • Coordinate and schedule appointments for clients, provide escort services to resources as needed.
  • Facilitates client enrollment in suitable schools and programs.
  • Monitors service delivery and progress.
  • Ensures compliance with NYS Office of Children and Family Services requirements by maintaining Family Assessment and Service Plans (FASP) and adheres to NYC Children’s Services (NYCCS) guidelines for service provision planning.
  • Gathers and processes relevant documents while accurately entering information into Connections and/or FASP.
  • Develops and presents service plans, providing updates to management, childcare staff, clinicians, and other relevant parties as needed.
  • Compiles necessary materials, including lists of approved visiting resources, schools, programs, and potential aftercare housing options.
  • Participates in meetings, including interdisciplinary team, monthly childcare/social services, weekly individual and group, and others as requested; occasionally prepares minutes or summaries of meetings to be documented in Connections.
  • Conducts field visits to outside organizations, such as schools, programs, and clients' family homes.
  • Contributes to planning of and participates in quality recreational events, including ethno-cultural activities and overnight out-of-state trips.
  • Provides or arranges for the provision of counseling services to residents and their families.
  • Diligently searches for missing residents (AWOC), files a missing person’s report with police, and obtains a warrant from a family court judge and files in the case record a copy of the police report and warrant. document efforts to find the missing resident in Connections.
  • Attends internal and external conferences, seminars, workshops, and in-service training sessions.
  • Initiates and maintains contacts internally and externally with collateral stakeholders as well as major stakeholders.

Requirements: 

 

  • ​​​​​Must be highly organized with the ability to efficiently coordinate work and schedules, as well as plan, organize, and lead case reviews and other meetings.
  • Must possess strong verbal and written communication skills, with the ability to convey information clearly and concisely, while effectively engaging with adolescents, colleagues, and external community resources.
  • Be flexible for occasional weekend and after-hours work, including urgent matters, planned activities, and potential overnight trips.
  • Have relevant experience or education in cross-cultural studies to enhance understanding of a diverse client population.

Qualifications


Qualifications:

 

  • Bachelor’s degree in human services or a related field preferred.
  • Must have two or more years' experience working in a human service field required.
  • Experience working with at risk youth in a foster care setting or prior foster care experience and knowledge of NYSDSS regulations and FASPs; familiarity with Child Welfare systems (NYCCS), including knowledge or resources available to the foster care population in general and to the severely emotionally disturbed (SED) and/or developmentally disabled.
  • Valid NYS Driver’s License preferred; not required.
