Resident Care Coordinator
Heartwood Place is seeking a Resident Care Coordinator, a full-time, purpose-driven position.
OVERVIEW OF POSITION:
As a manager in our RCC role, nursing services will be delivered to residents with the intentionality and attention to detail you provide. Med Techs will thrive under your leadership due to your excellent teaching and training, and you will be a valued member of our multi-disciplinary team. Collaboration with various teams of healthcare providers will be successful due to the effective communication skills and leadership you bring, with resident wellness being the forefront of your focus.
QUALIFICATIONS:
- Must be 18 years of age.
- Two Years Experience as a Med Tech or previous Resident Care Coordinator.
- Experience working in a senior living facility.
- Demonstrated knowledge of resident care processes, particularly as it relates to geriatric, long-term care, and dementia residents.
JOB DUTIES:
- Bring your A-game to the Care Team each day.
- Employ the basics: strong organization, written communication, time management.
- Actively evaluate and identify evolving health needs of our residents via resident assessments.
- Ensure attention to detail in implementing our processes with our Med Techs.
- Assist with teaching and training all care staff and Med Tech’s, i.e., build a team you are proud of!

Pay: $23.00 - $25.00 per hour
