Heaven Sent provides skilled home care services to clients in the Richmond, Petersburg, Hopewell, and Prince George areas. Heaven Sent is a privately owned and operated home health agency which has been in business for over 30 years. The organization is a deemed status health care agency accredited by the Joint Commission.

Position: LPN

The following statements are intended to describe the general nature of the level of responsibilities, duties, and skill required of individuals assigned to this job.

POSITION SUMMARY:

• Demonstrates leadership in providing skilled nursing services in the home.
• Carries out prescribed treatments, nursing care and health instruction
• Assist patients in reaching their potential rehab goals.

EDUCATION/EXPERIENCE:

• LPN with a minimum of 2 year recent Home Health experience.
• Current VA licensure.
• Understanding of computerized documentation systems.
• Must have excellent leadership ability, assessment and interview kills and knowledge of home health.
• Has an ability to deal tactfully with the community.
• Understands home care and the services provided to patient and family/caregiver through an interdisciplinary group.

CONTACTS:

Frequent contacts with patients under a wide variety of circumstances. Contact with family members in the patient’s home. Contact with staff and visitors in the office environment.

PHYSICAL DEMANDS:

Lifts, positions, pushes, and/or transfers patients. Considerable reaching, stooping, bending, kneeling, and crouching. Use of personal vehicle. Job entails traveling.

WORKING CONDITIONS:

Maybe exposed to the risk of blood borne diseases that require use of personal protective equipment; contact with patients under a wide variety of circumstances; exposed to unpleasant elements(accidents, injuries, illnesses); subject to varying home environments which may not be ideal, subject to varying and unpredictable situations in recipient’s home and agency office.
