Hegira Health, Inc. (HHI), a private non-profit corporation, is one of the largest freestanding behavioral healthcare agencies in the area. HHI, accredited by the Joint Commission and licensed by the State of Michigan, provides a broad array of mental health and substance abuse treatment and prevention services to individuals of all ages.
Oakdale Recovery Center (ORC) - provides withdrawal management and short-term residential treatment services to adults with substance use disorders, many whom have mental health disorders as well. Average LOS is 3 days for WD and 14 days for residential treatment. Hegira Health, Inc. The following shift is available:
- Full Time - Afternoon (3:00pm - 11:30pm) with rotating weekends
EDUCATION/EXPERIENCE:
- RN or LPN licensed to practice in the State of Michigan.
- A minimum of 1 year in the delivery of direct nursing care to adults with behavioral health disorders, direct experience in a psychiatric setting preferred.
- Knowledge of mental health and physiological and psychological effects of substance abuse as evidenced by a minimum of six (6) months experience in either outpatient, residential, or inpatient substance abuse treatment setting.
- Understanding of psychopharmacology, as demonstrated through academic, certification, and/or employment experiences.
PRIMARY RESPONSIBILITIES (Summary of):
Admission
- Reviews available historical information regarding scheduled admissions.
- Facilitates admission triage.
- Acts upon medical instability identified through triage, documents findings, and actions.
- Completes the health assessment and documents.
- Facilitates medical management of immediate needs per health assessment, e.g., TB testing, untreated infection, seizure risk, etc.
- Completes OOWS, COWS and/or CIWA during admission assessment process.
- Consult with medical staff regarding determination of level of care at admission.
- Faxes medication orders to pharmacy to ensure continuation of prescribed dosages.
- Initiates Medication Administration Record within 4 hours of admission.
- Initiates Detox Flow sheet within 4 hours of admission.
- Ensures Urine Drug Screen is completed on day of admission.
- Contacts or facilitates contact to laboratory for ordered tests.
- Writes Initial and Nursing Treatment Plan for completed health assessments.
Discharge
- Complete Medications, Labs and TB Skin Test, External Referrals, Physical Status, Medical Condition, Intervention and Response sections of Discharge Summary.
- Provide completed Medication Reconciliation Form to patient at time of discharge.
- Provide patient with prescriptions, medications, and physician orders at time of discharge.
Ongoing
- Maintain prioritized physician list for requested initial exams and follow-up care.
- Provide physician/PA/NP with verbal report of patient status/history upon arrival.
- Document each encounter of substantive value with a patient.
- Provide patient education regarding medication and health matters.
- Complete controlled substance count at each shift change.
- Administer medications at assigned times and document.
- Update Medication Administration Record consistent with physician/PA/PN orders.
Pay is based upon license (LPN or RN) and years of experience.
