What are the responsibilities and job description for the Peer Recovery Coach position at Hegira Health, Inc.?
Hegira Health, Inc. (HHI), a private non-profit corporation, is one of the largest freestanding behavioral healthcare agencies in the area. HHI, accredited by the Joint Commission and licensed by the State of Michigan, provides a broad array of mental health and substance abuse treatment and prevention services to individuals of all ages.
The Peer Recovery Coach (PRC) handles warm handoffs for new client referrals, internal referrals and stepdowns from Oakdale Recovery Center. The PRC will be responsible for signing clients up for outpatient peer service. Removing barriers to recovery, identifying goals and empowering clients to gain recovery capital are key goals for the position. Client meetings are held via telehealth, in the community or in office. Two positions available:
1) Inkster - Western Wayne Family Health Centers
- Monday through Friday 8:30am - 5:00pm
2) Livonia - and surrounding area, can meet clients in office or in the community
- Monday through Friday 9:00am - 5:30pm. Some flexibility required as needed to accommodate clients with a weekend or evening appointment.
EDUCATION AND EXPERIENCE REQUIREMENTS:
- High School Diploma or GED
- Must have completed Connecticut Community for Addiction Recovery (CCAR) training
- One year Peer Recovery Coach experience, experience working with individuals receiving Medication Assisted Treatment (MAT) as well as integrated care is preferred
- Current Peer certification issued by the State of Michigan, including certification or an active development plan through Michigan Certification Board for Addiction Professionals (MCBAP)
- Possess a Chauffer’s license (or obtain one at time of hire)
- Certified Peer Recovery Mentor (CPRM) Development Plan through MCBAP
RESPONSIBILITIES:
- Help to initiate and sustain individuals in recovery from substance abuse use disorder through a non-clinical, peer-based support program, following established HHI guidelines and processes.
- Complete face-to-face screenings for consumers using standardized screening tools.
- Complete documentation on all consumer contacts made or initiated.
- Complete Care Plans with consumers as needed.
- Assist consumer by making warm connections/transfers to services both internal and external.
- Work with consumers to identify barriers to successfully making changes and assist the consumer in problem solving.
- Monitor daily referral notifications and arrange same-day introduction meeting with consumers during shift.
- Facilitate contacts and provide warm transfers with appropriate support organizations, and support people to promote positive influences on consumers.
- Support the activities of other HHI staff in assisting the consumer with their treatment plan goals.
- Must be able and willing to drive to various locations around Wayne County for visitation of consumers in confidential locations (e.g. support groups, therapy).
Job Type: Full-time
Pay: $17.40 - $20.40 per hour
