Description
Under the direction of the Director of IDD Authority Services, the Habilitation Coordinator has the necessary authority to provide services to eligible individuals to provide assistance for an individual residing in a Nursing Facility to access appropriate specialized services necessary to achieve a quality of life and level of community participation acceptable to the individual and Legally Authorized Representative (LAR) on the individual’s behalf. The Pre-Admission Screening and Resident Review (PASRR) Habilitation Coordinator monitors Nursing Facility and LIDDA Specialized Services within a Nursing Facility setting. The IDD Authority Trainer and IDD Authority Team Lead assist Director in supervision and support for this position.
Responsibilities include, but are not limited to:
The Habilitation Coordinator (HC) meets face-to-face with an individual monthly, or more frequently if needed in the Nursing Facility to monitor progress/lack of progress on identified outcome goals, service delivery, and service satisfaction on the Habilitation Service Plan (HSP). The HC meets and facilitates a Service Planning Team (SPT) Meeting initially, quarterly, and annually to review assessments and individual’s need for specialized services, develop the HSP, review and monitor risk factors, make time referrals, service changes, and preferences to monitor if the individual is provided opportunities for engaging in integrated activities with residents of the Nursing Facility who do not have an Intellectual or Developmental Disability and in community settings with people who do not have a disabilitity. Annual Interdisiplinary Team Meetings (IDTs) are completed initially and on an annual basis. Discovery is completed with the individual and LAR on an ongoing basis. A Habilitation Assessment and Individual Profile are completed by the HC initially, as needs change, and annually therefater. Community Living Options reviews are completed every 6 months with the individual or LAR. The HC coordinates transitioning planning with the assigned Enhanced Community Coordinator (ECC) and Continuity of Care Coordinator (COCC) for an individual transitioning from the Nursing Facility to a community setting. The HC will utilize the Texas Medicaid Health Care Partnership (TMHP) program to enter and confirm assessments and PASRR Comprehensive Service Plan (PCSP). The HC will be a back-up to the PASRR Intake Coordinator in monitoring alerts in TMHP and completing PASRR Evaluations (PEs) if needed.
If the Habilitation Coordinator qualifies to provide Enhanced Community Coordination services, this position will complete both duties to assist the individual to transition to the appropriate desired community setting by either diverting from that setting or transitioning from that facility. ECC services will be provided for up to one year post transition or diversion for an enhanced service delivery system for pre and post move monitoring.
Service Coordination services for the Home & Community Based Services (HCS) program are provided of monitoring, service planning and coordination, assessment, and crisis prevention during this period of post-transition or diversion for up to one year before being assigned to a routine Service Coordinator and ECC services ending. These include a minimum of one monthly face-to-face (Type A) contact and three collateral (Type B) contacts for a monthly target per individual on assigned caseload.
Work requires, frequent initiative and independent judgement. The job requires extensive travel for community based service provision and flexible hours and schedule. The Center covers 19 counties for which flexibility and adaptability to state programmatic changes is required. Additional duties can be assigned based on departmental needs. Reassignment is a condition of employment that may occur at the employer’s discretion. Your work location and hours could change based on program needs. HFC incorporates principles of trauma-informed care, which includes cultural sensitivity, in all areas of service delivery. Qualifications
Education, Training, and Experience
Must have a bachelor’s or advanced degree from an accredited college or university with a major in a social, behavioral, or human service field including, but not limited to, psychology, social work, counseling, sociology, human development or educational psychology. Must have at least 1 year of experience working directly with persons with an intellectual disability or other developmental disabilities to be a Qualified Intellectual Disability Professional (QIDP). Must have at least two years’ experience providing Service Coordination to complete Enhanced Community Coordination (ECC) duties.
Must have current driver’s license and be insurable under Center insurance. Position may require use of personal vehicle in performance of job related duties and proof of PIP insurance. Must pass all Center mandated trainings.

- Ability to write progress notes according to standard, assessments, memos, letters, various complex reports and other documents with acceptable spelling and grammar.

- Skilled in the use of personal and network computers, word processing, database, spreadsheet and office management software (Microsoft Office), familiarity with CARE, TMHP, CSIL, SASO a plus.
