Summary of Essential Functions:
- Assists individuals served in making informed choices,
- Assisting/ empowering in advocating for physical and emotional needs.
- Completes Adult Needs & Strengths Assessments (ANSA), and other diagnosis specific assessments as needed/required.
- Updates Person-Centered recovery plans, &
- Completes progress notes in a timely manner.
- Meets individuals served at their home or in the community.
- Informs individuals served of community support services available.
- Crisis Support On-call consists of face-to-face assessment within the community
- Work within a Trauma informed environment for both Individuals served and Staff.
Schedule:
8am -5pm Monday to Friday on-call rotation
General Information:
Position requires initiative, excellent communication and organizational skills, and the ability to interact in a professional manner with a variety of staff members, consumers, family members, and community agencies. Work is performed with minimal supervision within established guidelines, policies, and procedures. Must attend all appropriate meetings. HFC incorporates principles of trauma-informed care throughout the agency, emphasizing cultural sensitivity and a safe and respectful environment for all individuals served and staff. All job descriptions include the responsibility to learn about and implement trauma-informed practices within the scope of work for the position. Reassignment is a condition of employment that may occur at the employer’s discretion. Your work location and hours could change based on program needs. Possibly subject to federal vaccination mandate, including provisions for claiming exemptions.
Qualifications
Qualifications:
- Bachelor’s degree (required) from an accredited college or university
- Driver’s license (required)
- Must be insurable to drive Center vehicles
