Description
Under the supervision of the Director of Child and Adolescent Services, the Family Partner is responsible for delivery of services as defined by the State authority, the Health and Human Services Commission (HHSC). Family Partners are active members of the case management/wraparound team providing peer mentoring and support to the primary caregivers.
Responsibilities include but are not limited to:
The incumbent schedules face-to-face encounters with clients and family members in accordance with individual needs and requires travel within Helen Farabee Centers’ catchment area. Peer mentoring, skills training, and support are provided by the Family Partner to the primary caregiver(s) of a youth who is receiving mental health services. This may include introducing the family to the treatment process; modeling advocacy skills; providing education and information; making referrals; providing parent skills training; assisting in the identification of natural and community supports. The Family Partner is the parent or legal authorized representative (LAR) of a youth with a serious emotional disturbance and has at least one year of experience navigating a child-serving system (e.g., mental health, juvenile justice, social security, special education) as the LAR to that youth. The Family Partner documents the provision of all Family Partner supports, both face-to-face and activities that ensure continuity of care that may or may not entail face-to-face contact.
Teamwork, initiative, and setting a positive example in work habits are expected at all times. Provides consultation and support to fellow staff as needed. This position requires an aptitude for working with severely mentally ill, often impoverished people, and for seeing this as work in which we take great pride. The incumbent must always be aware of the needs of youth in Child & Adolescent Services and provide supportive interventions and attend staff meetings and wraparound planning meetings. HFC incorporates principles of trauma-informed care throughout the agency, emphasizing cultural sensitivity and a safe and respectful environment for all individuals served and staff. Qualifications
Education, Training, and Experience
Graduation from standard high school or equivalent required. The Family Partner must have previous experience as a parent or primary caregiver of a youth with serious emotional disturbance. Experience with severe and persistent mental illness is required. Certification through the Texas Certification Board is required.
Must have a valid driver’s license and be insurable to drive center vehicles. Must pass all center-mandated trainings.
Knowledge, Skills, and Abilities
- Ability to read and proof correspondence, reports and computer printouts manually, etc.
- Good spelling, grammar, and general writing skills
- Ability to write service notes according to standard with acceptable spelling and grammar.
- Display a customer service orientation at all times treating clients as equals and partners.
- Ability to relate verbal instructions, communicate in a courteous, and effective fashion with co-workers, patients, families, agencies and the public.
- Skilled in the use of network computers, word processing, database software, SmartCare, as it relates to documenting client and family contacts.
