Under the direction of the Director of IDD Authority Services, the Intake Coordinator serves as the identified point of contact for individuals, families, professionals, and agencies seeking information regarding Intellectual Developmental Disability (IDD) services in the 19-county local service area of the Helen Farabee Centers. The Authority Team Lead is the secondary supervisor for this position for which the Intake Trainer also provides collaborative training and oversight with Director as the tertiary supervisor.

Responsibilities include, but are not limited to:

Completes intakes for IDD services timely and makes referrals within the organization or to other agencies that can meet the individuals needs. Collaborates with various agencies to obtain records to help support the determination of eligiblity process with the psychology department for program services. Maintains a centralized comprehensive interest lists waiting lists for IDD services in accordance with Texas Health & Human Services Commission (HHSC) and LIDDA standards and procedures for the Home and Community Based Services (HCS) Medicaid Waiver, Texas Home Living Medicaid Waiver, and General Revenue (GR) programs. Conducts biennial contacts with individuals or their primary correspondent who are listed on the interest lists. Serves as the transition contact person for the LIDDA and completes referrals based on needs. Promotes advocacy and participates with the Community Resource Coordination Groups (CRCGs) for individuals with muti-service agency needs and Admission, Review & Dismissal (ARD) meetings within schools in the LIDDAs local service area. Completes initial enrollments for Community First Choice (CFC) non-waiver assessments and processes for individuals 21 years and older, data entry in the statewide data system for ID/RC level of care assessments for CFC non-waiver and CFC STAR KIDS programs under 21 years of age. Attends community educational events in the LIDDAs service area as scheduled and needed to educate the community and agencies on IDD programs and services. Ensures all initial assignment packets are completed timely for service monitoring continuation to occur with assigned LIDDA staff.

Travel required in 19 county area. Work requires frequent initiative, independent judgement, program planning and interagency coordination. Work requires the ability to adapt to state and local changes that occur over time due to funding and policy changes. HFC incorporates principles of trauma-informed care throughout the agency, emphasizing cultural sensitivity and a safe and respectful environment for all individuals served and staff. All job descriptions include the responsibility to learn about and implement trauma-informed practices within the scope of work for the position. Reassignment is a condition of employment that may occur at the employers discretion. Your work location and hours could change based on program needs.

Education, Training, and Experience

Must have bachelors or advanced degree from an accredited college or university with a major in a social, behavioral, or human service field including, but not limited to, psychology, social work, counseling, sociology, human development, or educational psychology. Prior experience working with individuals with disabilities preferred, but not required. Must have current drivers license and be insurable under Center insurance. Position may require use of personal vehicle in performance of job-related duties and proof of PIP insurance. Must pass all Center mandated trainings.

Knowledge, Skills, and Abilities

  • Knowledge of IDD, rehabilitation methods, basic casework principles, Person-Centered Planning, and general community welfare resources. Demonstrated skill in interviewing and reporting results of interviews; ability to make appropriate referrals.
  • Basic understanding of mathematics is required. Ability to calculate amounts due for services.
  • Ability to write effectively, prepare reports and make recommendations for administrative use.
  • Must be able to provide verbal feedback of reports to individuals, families, staff, and participate verbally in meeting with community agencies and give verbal presentations.
  • LIDDA/Local Authority IDD Service Delivery System
  • Must possess the ability to use basic word processing equipment and programs, spreadsheets, and work in data base programs to complete data entry.
  • Ability of use copy machine, fax machine and calculator. CARE, TMHP, CSIL, SASO, SFTP, SmartCare a plus.
