Helio Health is on a mission to treat and promote recovery from the effects of substance use, mental health disorders, and other behavioral healthcare issues. To support the mission of Helio Health, the Adult Health Home Team Leader will provide supervision and day to day operations of the adult health home program. The Team Leader will work on a Full-Time basis in the UTICA area. Travel is required to Syracuse twice per week.

Pay Rate: $27.00 - $29.72 per hour
- Maintains and provides supervision of program staff.
- Participates in Management and Administrative committees as assigned.
- Assures program record documentation is in compliance with agency and regulatory standards.
- Represents agency to the community in multiple counties
- Occasionally carry a small caseload as needed for coverage purposes.
- Other duties as assigned by supervisor.
- Minimum three years of health home experience in an appropriate program setting.
- Minimum Bachelor’s Degree, Master’s Degree in a related human service field strongly preferred.
- Previous supervisory experience preferred.
- Must have a valid driver's license.
