What You'll Do
As a Diversion Specialist for one of our women’s assessment shelters, your primary goal will be to help your clients by working closely with HRA, Homebase, and, in the case of Veterans, SSVF to either prevent the loss of an apartment or to rapidly rehouse your client. You'll be responsible for providing services and referrals including assessments, counseling, service planning, and entitlements. Services and referrals should ensure the expeditious diversion from shelter or placement of individuals into permanent housing which is consistent with the goal of assisting individuals to become self-sufficient.
Your responsibilities will include:
Conducting initial intake interviews and assessments for all new residents, establishing and maintaining case files with proper documentation, and reviewing site specific policies and procedures with all new residents.
Formulating proper intervention based on differential assessment of the resident’s situation – quickly assessing contributing factors of homelessness and tailoring intervention accordingly.
Providing service referrals for Homebase, SSVF, and other partner agencies and serving as liaison with various agencies.
Providing mediation assistance and conflict resolution with the goal of family reunification where appropriate.
Advocating for and assisting individuals in negotiation of the service delivery system, ensuring receipt of appropriate entitlements while remaining connected to attainment of permanent housing.
Engaging clients in problem solving dialogue to identify and assess additional resources a client may be able to access.
Providing leadership and direction in the area of housing for all residents.
You’re a great fit for this role if you have:
Bachelor's degree in social work or a related field OR equivalent experience and skills.
Prior experience in case management, assessment, counseling and crisis intervention.
Knowledge and understanding of Adult Systems approach to practice and team concepts.
Ability to provide evening and weekend coverage as needed.
