Responsibilities
- Assist individuals with daily living activities, including personal hygiene, meal preparation, and medication management.
- Provide companionship and emotional support to enhance the social well-being of clients.
- Monitor and document patient progress, reporting any changes or concerns to the appropriate personnel.
- Facilitate community engagement by accompanying clients to appointments, social events, or recreational activities.
- Maintain a clean and safe environment for clients, ensuring all areas are organized and free from hazards.
- Collaborate with healthcare professionals and family members to develop and implement individualized care plans.
- Support clients in achieving their personal goals through skill-building activities and encouragement.
- Perform heavy lifting as necessary to assist clients with mobility or transfers.
Requirements
- Experience in home health, long-term care, or nursing home settings is preferred.
- Familiarity with patient care techniques and patient monitoring practices is essential.
- Strong interpersonal skills with the ability to work effectively with individuals with disabilities.
- Background in social work or related fields is a plus.
- Ability to perform heavy lifting and assist clients with mobility needs.
- Must possess a compassionate demeanor and a commitment to providing high-quality care.
