Peer Support Specialist/ Case Manager
Position Description:
The Case Manager or Peer Support Specialist at The Haven will conduct individual case management or Peer Support Services to assist our Residential clients. Our main goals are to start our clients on the road to recovery and lastly provide them with tools for self-sufficiency.
Hours:
Maximum 40 hours/week, Monday – Friday.
Location of Work:
West Jordan, UT
Requirements:
- 18 years of age
- Ability to pass a background check
- CPR & First Aid certification (to take once hired)
- TB test (to take once hired)
- Valid Drivers License with less than 3 infractions in the last 3 years
Duties:
- Conduct & document needs assessments
- Collaborate on goal setting
- Develop written, individualized case management service plans with specific outcomes
- Assist with wrap around services
- Conduct Peer Support Groups
- Develop and maintain clear, organized case management documentation
- Maintain compliance with any agency contracts, policies and employment guidelines
- Attend community events as needed
- Attend all relevant staff and community meetings
- Other duties as assigned
Job Type: Full-time
Pay: $18.00 - $19.00 per hour
Expected hours: 40 per week
