Social Worker
Our center has been serving the elderly 65 years and above, and adults with chronic, medical, and cognitive or mental health conditions for over 25 years. We are looking for an MSW to join our team in working towards the company goal of promoting and restoring participant health.
Position Summary:
The Social Worker will work closely with participants to assess their needs, develop plans of care, and provide supervision and supports to help clients.
Responsibilities:
·Develop individualized care plans
·Maintain records of client progress notes, discharge planning records, and discharge planning reports
·Assist with processing admission and enrollment of participants
·Maintain confidentiality of client records according to HIPAA regulations
·Educate families and participants in regards to health and prevention of health decline
Preferred Languages: English, Tagalog, Spanish
Job Type: Full-time
Pay: $30.00 per hour
Schedule:
- 4 hour shift
- 8 hour shift
- Day shift
- Monday to Friday

- Social work: 1 year (Preferred)
