The Qualified Professional/Intake Coordinator will provide administrative support in-office and is responsible for processing referrals, intakes, scheduling of service/treatment with clinician, coordinate walk-ins, assign caseload, provide client and staff orientation and provide additional administrative support to the team.

Responsibilities:
- Ensure continuity of care for person served including authorizations, annual documentation to support ongoing maintenance of services
- For each client assigned, provide an orientation that outlines a goal statement, explain and schedule services and clarify the following: confidentiality, client rights, consents for services, and financial requirements.
- Provide Person Centered Planning/treatment planning by preparing the initial diagnosis; describing strengths, preferences and needs; and identifying treatment goals and strategies for each client assigned. Complete and document the treatment/service plan by the time the client enters the service.
- Monitor adherence to plan of care including utilization and service provision
- Communicate proactively with individuals served, family members, and other stakeholders to solicit input and feedback on the quality of care
- Document progress and submit documentation in a timely manner
- Receive referrals and effectively communicate with adjusters, supervisors, marketing team and case managers to process referrals
- Answer incoming phone calls and route to appropriate staff
- Process case closures, case transfers, reopened cases, and closure surveys
- Review, interpret, compile and send medical record requests
- Document progress notes for clients assigned within one working day of service, comply with information guidelines and signature requirements, maintain chart organization, and file in a timely manner.
- Assist Human Resources with onboarding and new hire orientation
- Is reliable in work attendance and follows Freedom House procedures when requesting planned or unplanned leave
- Maintain confidential and proprietary information
Skills:
- Write Person-Centered Plan (PCP) and Crisis Plan and already completed the DHHS required training: From Theory to Practice: Person-Centered Planning in NC
- Familiarity with submitting SAR via the various MCO & PHP portals.
- Ability to communicate professionally (oral and written)
- Advanced Microsoft Word and Excel skills
- Maintain business acumen and professional demeanor in-person and telephonically
- Excellent multitasking, organizational, and time management skills
- Demonstrated diplomacy with internal and external customers
- Independent judgment, problem solving, and decision-making skills
- Adaptable and flexible when completing projects with frequent shifts in direction

Experience:
Must meet requirements specified for Qualified Professional per North Carolina Administrative Code and have a minimum of 2 years’ experience in child, adolescent and adult mental health/substance abuse treatment services.
Job Types: Full-time, Contract
Pay: $18.00 - $25.00 per hour
Expected hours: 40 per week

- Monday to Friday
- No weekends
