Helping Hands of New Orleans, LLC is a home and community based service provider.
We are currently searching for a Facilitator to join our team. We currently service individuals in the Metropolitan Area. (Region 1).
JOB OVERVIEW:
The Facilitator is responsible for coordinating comprehensive services for individuals that we service. The Facilitator has direct oversight and responsibility for all residential services provided to people HHNO serves. The Facilitator will maintain a caseload and work closely with all team members to ensure that each individual’s plan is implemented. The facilitator works closely with the Program Director and other Support Team Leaders to ensure Direct Support Professionals receive training and build the necessary skills to successfully support the individuals the agency serves.
JOB RESPONSIBILITIES:
Job responsibilities include but is not limited to:
- Actively participate in the person-centered planning process, including but not limited to: attending all planning meetings, providing input, coordinating the development of training plans, ensuring all necessary documentation is completed, reviewing progress notes and daily notes, monitoring for the implementation of the plan, completing monthly reviews, and communication with service coordinators.
- Coordination and oversight of medical appointments, including attending appointments if necessary.
- Face to face visits with individuals in their homes, ensuring that each individual’s rights are respected and protected, personal plan objectives are being implemented, and issues are resolved in a timely manner.
- Monitoring for consumer safety, including ensuring that each home environment is safe, clean, and in good repair.
- Coordination and oversight of maintaining current data in the individual’s record, including but not limited to: individual profile, emergency contact numbers, protected health information, release and consent forms, daily progress notes, monthly reviews, MARS and physician orders. Ensuring staff compliance with the Health Insurance Portability Accountability Act of 1996 (HIPPA).
- Participation in incident management, including but not limited to review of all Critical Incident Reports, making appropriate and required notifications, and oversight of follow up to all incidents.
- Maintain positive communication with family members, guardians, staff, and Office Personnel about policies, issues, and/or concerns.
- Prepare and submit applicable reports and documentation related to individuals supported by HHNO.
- Provide supervision and leadership to companions and direct support professionals for implementation of the person-centered plan, identifying community resources, and assisting individuals in being active participants in their respective communities.
- Be available and responsive to companions and direct support staff in emergency and crisis situations, including on call duties.
- Performs other related work, as assigned by the Director.
- Ensure compliance with DHH guidelines.
- Manage growth of residential services in such a way that we are meeting budgetary expectations while also maintaining quality services to those we serve residentially. Lead agency through development and implementation of strategic plan and goals.
QUALIFICATIONS:
- Bachelor’s Degree in a human service related field (Master’s Degree Preferred).
- Minimum of 4 years working with elderly and persons with developmental disabilities
- At least 25 years of age
- Must have a valid Louisiana Driver's License and access to an insured vehicle
- Current CPR (preferred)
- Must be able to pass a background check
