Job Summary
We are seeking a compassionate and dedicated Residential Program Specialist to join our team in providing exceptional care to individuals with intellectual and developmental disabilities in a residential setting. We serve 14 residents in 6 community group homes throughout Pottstown, Gilbertsville, and Perkiomenville.
Responsibilities
Job responsibilities can include but are not limited to:
- Preparation, implementation, and monitoring of all components of individual assessments, individual support plans (ISP), lifetime medical history, and annual packet.
- Attend all individual team meetings, staff meetings when appropriate, as well as annual and quarterly meetings.
- Prepare data sheets and/or other requested forms related to documentation of individual progress.
- Training DSP staff how to implement individual outcomes and behavior modification techniques.
- Train staff on applicable documentation software, along with proper completion of all necessary data to be entered into the system.
- Review ISP data monthly in order to track progress and ensure proper application, documentation, and implementation.
- Assist with monitoring plans prepared by outside consultants. (i.e.: behavioral supports, as well as physical/occupational/speech/sensory therapies)
- Prepare quarterly and annual reports for each individual on assigned caseload.
- Visit each residential site routinely to establish relationships with individuals and staff.
- Oversight, filing, and purging of all pertinent licensing books.
- Participate in the on-call rotation, along with providing coverage when necessary, especially during times of staffing crisis.
- Cover direct care shifts as needed.
- Collaborate with healthcare professionals, families, and other staff members to develop personalized care plans for each resident.
- Document resident progress and any incidents accurately in compliance with organizational policies.
Requirements
- Demonstrates knowledge of and adheres to organizational policies, procedures, and guidelines, as well as ODP licensing regulations.
- Associate degree (min 60 credit hours) in Human Services Field. (ie, Psychology, Education, Social Work, Sociology) Bachelor degree preferred.
- Minimum of 3 years experience working directly with individuals diagnosed with IDD/A and Mental Health disorders required. 2 years program specialist experience preferred but not required.
- Strong communication skills and the ability to work effectively within a team environment.
- Compassionate nature with a genuine desire to improve the lives of individuals with developmental disabilities.
- Must be able to pass background checks as required by state regulations.
