Home Care Nurse

We are seeking a compassionate and skilled Home Care Nurse to provide high-quality care to patients in their homes. The ideal candidate will have experience in homecare nursing and be able to work independently while also collaborating with other healthcare professionals.

Responsibilities:

• Assess patients' health status and develop individualized care plans
• Administer medications and treatments as prescribed by physicians
• Monitor patients' vital signs and report any changes to the healthcare team
• Provide wound care and other nursing interventions as needed
• Assist patients with activities of daily living, including bathing, dressing, and grooming
• Educate patients and their families on disease management and self-care techniques
• Document patient care and progress in electronic medical records
• Collaborate with other healthcare professionals to ensure coordinated and comprehensive care

Requirements:

• Active nursing license in the state of practice
• Minimum of 1 year of experience in homecare nursing
• Excellent communication and interpersonal skills
• Ability to work independently and manage time effectively
• Strong critical thinking and problem-solving skills
• Comfortable working with patients of all ages and backgrounds
• Valid driver's license and reliable transportation
