HYC has an immediate opening for a full-time (ECM) Lead Care Manager who will be responsible for providing care to members that promotes resilience and support, lifestyle changes, member skill development and self-management.
Role Duties
- Provides multiple strategies for engagement, as appropriate and to the extent possible, including direct communications with the Member. Engagement is offered through in-person meetings where the Member lives, seeks care, or is accessible by either mail, email, texts and telephone, community, and street-level outreach.
- Provides an active and progressive approach to outreach and engagement until the Member is engaged.4
- The ECM collaborates and communicates with clients, and other members of their “Care Team” such caregivers/family support persons, and other providers such as primary care physicians.
- Provides culturally and linguistically appropriate communication and information to engage Members.
- Responsible for developing a comprehensive assessment and care plan that is individualized and person-centered which coordinates and integrates all the Member's clinical and non-clinical health care related needs.
- The ECM will coordinate health care services, provide health education, and facilitate care in a timely manner.
- Develop strategies to reduce avoidable Member admissions and readmissions and provide adherence support and referral to appropriate services.
- Maintain accurate and timely documentation of all job-related activities.
- Coordinate with external care providers and entities providing the member with some level of care.
- Coordinate transportation and accompany members to critical appointments as needed.
- Will be required to participate in required training and clinical supervision.
- Must have excellent organizational skills and must complete all required reports and documentation in a timely manner.
- Participate in weekly clinical supervision or case conferences that may include other members of interdisciplinary team.
- Complete weekly case logs/outreach logs provided in Excel.
- Performs other duties as assigned by supervisor.
Qualifications
REQUIREMENTS:
- Bachelors’ degree in the behavioral sciences.
- One (1) year's full-time work experience in community-based services field.
- Permanent U.S. residency or citizenship.
- Ability to pass criminal clearance background check.
- Ability to successfully pass a TB screening test.
- Proof of U.S. citizenship or permanent residence.
- Willingness to work a flexible schedule, including evenings.
- Current auto insurance, and a good driving record.
- Valid California driver's license, current auto insurance, a good driving record, and must be cleared to drive for business purposes by our insurance broker.
- Ability to manage multiple tasks, projects and priorities.
- Excellent administrative and verbal skills.
PREFERRED QUALIFICATIONS:
- Experience in working within the Southeast Region of Los Angeles County and a familiarity with the population of the area.
- Experience in networking with school and community agencies in providing client care services programs.
- Experience in working in a community-based organization.
- Excellent verbal and written communications skills preferred in both English and Spanish
