Title: Pathway Home Case Manager
Monday-Friday: 8am-5pm
Under the supervision of the Pathway Home Program Manager, the Case Manager provides high quality case management services to clients residing at the Pathway Home site in East Los Angeles. The case manager shall serve as the central point of contact for clients, as they navigate the transition from person experiencing homelessness to tenant living within the community. Specifically, they will:
RESPONSIBILITIES:
- Conduct initial and quarterly housing/ activities of daily living assessments.
- Develop and implement an individualized care plan, in collaboration with the client, based on the housing acuity assessment and activities of daily living assessment.
- Write detailed case notes in G.I.R.P. format
- Maintain updated calendar with client visits and times
- Develop professional relationships with clients and maintain a case load of 30 participants.
- Collaborate with permanent housing and housing navigation providers to support the transition of participants to permanent housing
- Conduct regular face-to-face check in meetings with clients
- Ensure that clients are linked to health, mental health, and substance use services as needed.
- Assist clients with obtaining employment, and/or establishing benefits, and/or educational opportunities.
- Assist clients with maintaining medication and treatment regimens including accompanying clients to appointments with health, mental health, and other care providers.
- Utilize a harm reduction, client focused and strengths-based model.
- Provide ongoing case management services that will ensure successful permanent housing, decrease social isolation, and prevent relapse risks.
- Respond to urgent requests for assistance from clients
- Participate in all meetings with clinical supervisors, collaborative agencies, and other community meetings as directed.

Qualifications
KNOWLEDGE, SKILLS AND ABILITIES:
- Knowledge surrounding poverty, homelessness and social factors involved.
- Ability to understand the needs of formerly homeless people with disabilities and to develop collaborative goals towards greater self-sufficiency and independence in the greater community.
- Ability to work flexible schedule, including some overtime as needed.
- Knowledgeable about services and resources for homeless and low-income individuals throughout Los Angeles County.
- Knowledge of the dynamics of chemical dependency, mental disorders and the effects of homelessness and ability to conduct workshops for residents on these subjects.
- Bilingual in English/Spanish preferred.
- Skilled in use of Microsoft Windows, Microsoft Office (Word, Excel, Access, PowerPoint, and G-Mail), and HMIS Homeless Management Information System.
- Must demonstrate excellent written and verbal communication skills.
REQUIRMENTS:
- High School Diploma or GED (required)
- Bachelor's degree in behavioral health and/or a minimum of 2 years of case management and outreach directly tied to homelessness.
- Possess a valid California driver's license and have access to a properly registered vehicle.
- Ability to pass a background check and criminal clearance
