What are the responsibilities and job description for the Mesa View Assisted Living Coordinator position at Hemphill County Hospital?
Overview: The Mesa View Assisted Living Coordinator oversees all aspects of the community's operations and performance, ensuring exceptional resident care and services while adhering to budgetary guidelines and service standards. This role ensures compliance with federal, state, and local regulations, as well as Mesa View Assisted Living policies and procedures, to maintain a high-quality, welcoming environment for residents and staff.
Essential Duties and Responsibilities
- Proactively promote the assisted living community by building positive relationships with residents, families, hospital discharge planners, social workers, local businesses, community organizations, healthcare professionals, and other referral sources.
- Foster regular and open communication with Mesa View residents and staff.
- Oversee and evaluate all community operations and staff functions, including dining services, health and wellness, marketing and admissions, housekeeping, laundry, social activities, transportation, and maintenance.
- Ensure a high-quality health and wellness delivery system through consistent review and monitoring of services offered.
- Conduct daily oversight of resident care and community engagement.
- Stay informed about federal, state, and local regulations, ensuring compliance and renewal of licenses, permits, and certifications.
- Adhere to and enforce safety rules and regulations, recommending necessary actions to supervisors as needed.
- Protect resident confidentiality at all times.
- Assist in the planning and development of the Mesa View operating budget, ensuring operations align with budgetary guidelines.
- Regularly monitor community performance, addressing corrective actions with HCH Administration as needed.
- Maintain accurate financial records and manage inventory for the Mesa View community.
- Oversee hiring, retention, and termination of staff, with support from HCH Human Resources.
- Ensure all employees receive comprehensive orientation and understand the needs of Mesa View residents.
- Address resident concerns regarding staff or employee-related issues in a timely and effective manner.
- Supervise, train, develop, and evaluate employees, providing ongoing feedback and performance reviews.
- Monitor employee appearance and enforce standards for personal hygiene and health.
- Attend required continuing education functions as mandated by the State of Texas.
- Follow aseptic procedures and take precautions during direct resident contact or when handling potentially infectious materials.
- Perform other related duties as assigned.
Other Requirements
- Document work processes in compliance with HCHD policies.
- Complete additional tasks as assigned by the supervisor or designee.
All duties and requirements must be performed in alignment with HCHD performance standards.
Education:
- Minimum: High School Diploma or GED equivalent.
- Preferred: Associate's Degree in nursing, healthcare management, or a related field.
Work Experience:
- Minimum: At least one year of experience in management or healthcare industry management.
Licenses & Certifications:
- Minimum:
- Maintain BLS certification as a condition of employment.
- Obtain assisted living community certification as required by the Department of Aging and Disability Services.
Physical Demands:
- Medium Physical Demand Level: Ability to lift up to 50 pounds, with frequent lifting and/or carrying objects weighing over 25 pounds.
- Duties Include:
- Extended periods of standing and walking throughout a 12 hour shift.
- Repeated bending, stooping, twisting and reaching.
- Pushing and pulling up to 50 pounds and carrying up to 50 pounds with limited assistance.
- Lifting up to 50 pounds, with heavier lifting involved in transferring or supporting patients who may suddenly become weak or helpless.
