JOB SUMMARY |
- Transitional care pharmacists are an integral component of the multidisciplinary team that cares for a patient during their stay at Hendrick Medical Center. The pharmacist is the bridge between outpatient and inpatient care by facilitating a safe and effective transition into the hospital system or discharge to outpatient services. A successful admission is accomplished by obtaining an accurate medication list from the patient and various trusted sources to ensure that hospital providers have the best information to treat the patient. During the patients stay transitional care pharmacists monitor patient’s progress to assist their team in preparing for discharge. Once discharge plans have been established, pharmacists work with the outpatient pharmacy to fill all discharge medications and assist with any barriers that might arise, such as the need for financial assistance. Pharmacists duties may include working with providers to ensure that all necessary medication is prescribed for discharge then meeting with patients to educate them on their medication regimen before leaving the hospital. Additionally, transitional care pharmacists have the opportunity to work in many different areas of the hospital as well as several specialty outpatient clinics.

- Required Licenses/Certifications
- Active license as registered pharmacist in the state of Texas
- Required Skills, Knowledge, and Abilities
- Ability to effectively direct and/or supervise personnel as appropriate
- Ability to handle confidential information
- Ability to maintain professional appearance and decorum at all times
- Ability to demonstrate excellent oral and written communication skills
