This position focuses on supporting and enhancing comprehensive wraparound services for medication-assisted treatment (MAT), addressing opioid use disorder (OUD), alcohol use disorder, and co-occurring mental health conditions.Current Hennepin County employees who refer a candidate hired into an open competitive position may be eligible for a $500referral bonus. Location and hours : This position is primarily located at the NorthPoint Health & Wellness Center at 2220 Plymouth Avenue North, Minneapolis, MN 55411 and in the Hennepin County community providing outreach. Work hours for this position are Monday through Friday, with occasional nights and weekends, from 8 : 30 a.m. - 5 p.m. New Employee Orientation (NEO) for all new benefit-earning employees is in-person in downtown Minneapolis. Details about NEO will be provided upon hire.About this position type : This is a full-time, benefit-earning position.This position is internally classified as a Community Health Worker, Senior.Click to view the job classification specification.In this position, you will : Provide care coordination for individuals initiating or maintaining Medication-Assisted Treatment (MAT), ensuring seamless access to medical, behavioral health, and human services. Assist patients with scheduling appointments, accessing transportation, and navigating community resources. Conduct follow-ups to ensure continuity of care and address barriers to recovery. Participate in care consultations with other members of the care team.Conduct outreach in the community to identify individuals in need of SUD and OUD services, focusing on underserved populations. Collaborate with local organizations, faith-based groups, and community leaders to raise awareness of available services. Represent the program at community events and forums to promote harm reduction and MAT services.Provide education to patients and families on harm reduction, MAT, and available resources. Advocate for patients by helping them navigate healthcare systems and addressing systemic barriers. Maintain accurate documentation of patient interactions and services provided in the electronic health record (EHR). Collect and report data on outreach and service outcomes to inform program improvements. Need to have : One of the following : Completion of two years or more of college in social / behavioral sciences or another field related to the area.Two years or more of experience working in a community outreach or public health setting.A valid driver's license, access to reliable transportation to travel to client's homes within Hennepin County, and the ability to obtain a Hennepin County driver's permit. Nice to have : Strong connection to and / or familiarity with the North Minneapolis community.Experience : Performing face-to-face outreach within diverse communities.Working in a community outreach or public health setting or another area related to substance use disorders.Providing care coordination for individuals initiating or maintaining Medication-Assisted Treatment (MAT).Maintaining accurate documentation in the electronic health record (EHR).Knowledge of harm reduction and trauma-informed practices.Ability to : Establish and maintain effective working relationships with staff, community agencies, and culturally / socioeconomically diverse clients.Communicate information to people with different levels of understanding and diverse backgrounds.Candidates with fluency in other languages in addition to English are encouraged to apply.
