Assist patients with scheduling appointments, accessing transportation, and navigating community resources. Conduct follow-ups to ensure continuity of care and address barriers to recovery. Participate in care consultations with other members of the care team.Conduct outreach in the community to identify individuals in need of SUD and OUD services, focusing on underserved populations. Collaborate with local organizations, faith-based groups, and community leaders to raise awareness of available services. Represent the program at community events and forums to promote harm reduction and MAT services.Provide education to patients and families on harm reduction, MAT, and available resources. Advocate for patients by helping them navigate healthcare systems and addressing systemic barriers. Maintain accurate documentation of patient interactions and services provided in the electronic health record (EHR). Collect and report data on outreach and service outcomes to inform program improvements. Need to have : One of the following : Completion of two years or more of college in social / behavioral sciences or another field related to the area.Two years or more of experience working in a community outreach or public health setting.A valid driver's license, access to reliable transportation to travel to client's homes within Hennepin County, and the ability to obtain a Hennepin County driver's permit.
