Description

Join us as we change the way mental health care is provided to those who need us most. We are seeking a Psychiatric Nurse Practitioner to provide mental health care at local shelters and drop-in centers serving homeless clients in the downtown Minneapolis area. In this role, you will primarily assess and provide ongoing psychiatric treatment for homeless persons with mental illness. You will work with a team of medical providers, registered nurses, social workers, licensed practical nurses, medical assistants, and case managers.

Location and hours :

This position is hybrid and will be performed both on-site at various Health Care for the Homeless (HCH) sites, throughout the community at shelters and drop-in centers to provide mental health services, and remote as job duties require. Work hours are primarily days, Monday through Friday, to be allocated as needed to fill in for staff, with some evening hours required. No weekend, overnight or holiday shifts are required. If possible, the candidate would be able to have at least one set workday a week, but this is flexible. While this position is designated as hybrid, based on current requirements hires must reside in or relocate to Minnesota or Wisconsin.

In this position, you will :

• Perform initial and ongoing assessments of patients' psychiatric and psychosocial status
• Provide ongoing medication management for patients including assessing effectiveness and adjusting medications for optimum effectiveness with minimal side effects; consult with other providers and team members
• Provide direct care, monitoring, and / or referral for co-occurring physical / medical conditions
• Provide consultation to medical providers on psychiatric patient concerns
• Serve as an active team member of a multi-disciplinary team by attending meetings; actively participating in program planning, development and evaluation; and assisting with reports and special projects

Need to have :

Possession of a valid license as a Registered Nurse issued by the State of Minnesota and National Certification as a Nurse Practitioner (CNP).

Nice to have :

Providing trauma-informed care.
