Description

The Criminal Justice Behavioral Health division of Behavioral Health is looking to hire a Senior Social Worker to support individuals at the Adult Correctional Facility (ACF) in Plymouth, MN. The residents in the ACF have various needs within the facility and this Senior Social Worker will offer support to the residents as they are released into the community. The ideal candidate for this role will be well organized, facilitate timely communications with providers, ACF staff, probation, and other community supports. This individual will join a team of four Senior Social Workers in this role.

Behavioral Health is an innovative and growing service area within Hennepin County's Human Services department. We provide a range of mental health and substance use services to residents including populations intersecting children, adults, and criminal justice. We have 230 staff including social workers, psychologists, program managers, supervisors, planners, and others working collaboratively to meet our system goals of expanded access, upstream help, and person-centered services.

Location and hours :

This position is hybrid and will be performed both onsite at the ACF at 1145 Shenandoah Lane, Plymouth, MN 55447, in the community, and remote as job duties require. The ACF is a medium level security facility, it is located 13 miles from downtown Minneapolis, and there is no public transportation to this facility. Work hours will be primarily days, Tuesday through Friday (10 hours per day), between the hours of 7 a.m. to 5 p.m. with some evening hours to facilitate groups in the facility. While this position is designated as hybrid, based on current requirements hires must reside in or relocate to Minnesota or Wisconsin.

In this position you will :

  • Interview residents and complete assessments for service needs and supports.
  • Set up case plans.
  • Create a transition plan that will support the resident and other parties, supporting the resident in understanding the plan, resources, and services.
  • Work with residents on resources, services and skill building.
  • Support running cognitive skills groups.
  • Engage with various partners in the ACF.
  • Work the residents in the community upon their release
  • Work with individuals to develop goals and service plans, make referrals to community services, and connect individuals to current supports.
  • Assist with forms and applications for benefits and services, determine eligibility, and provide information on community resources.
  • Work closely with individuals in the community, local law enforcement, probation officers, corrections staff, attorneys, community providers, and others.
  • Complete paperwork and documentation utilizing county and court systems.
  • Provide consultation and support to teammates including case consultation, signing documentation, facilitating workgroups or meetings, and / or developing tools and resources.

Need to have :

  • One of the following :
  • Bachelor's degree in social work, psychology, sociology, or human services, and 18 semester (27 quarter) credits of graduate work in the above areas, and three years or more of full-time supervised experience as a social worker.

  • Master's degree or higher in social work or closely related field with course work emphasis in areas such as social work methods, human growth and behavior, and principles of social welfare.
  • Licensed as a Graduate Social Worker by the Minnesota Board of Social Work.
  • A valid driver's license, daily access to reliable transportation, and the ability to obtain a Hennepin County driver's permit to travel primarily within / outside of the metro area to meet with clients in their residential settings and / or perform authorized off-site work duties.

  • Licensed Alcohol and Drug Counselor (LADC), Licensed Graduate Social Worker (LGSW), Licensed Associate Marriage and Family Therapist (LAMFT), Licensed Marriage and Family Therapist (LMFT), Licensed Professional Counselor (LPC), Licensed Independent Clinical Social Worker (LICSW), or Licensed Professional Clinical Counselor (LPCC).
