What are the responsibilities and job description for the Trainee Case Management Assistant (Dari and Pashto bilingual) position at Hennepin County?
This position is hybrid and will be based at the South Human Service Center at 2215 East Lake Street, Minneapolis MN 55407, in the community with direct contact with residents, and remote as job duties require. Work hours will vary between 8 a.m. - 5 p.m., Monday through Friday, with occasional evening and weekend hours. While this position is hybrid, based on current requirements hires must reside in or relocate to Minnesota or Wisconsin. New Employee Orientation (NEO) for all new benefit-earning employees is in-person in downtown Minneapolis. Details about NEO will be provided upon hire.

- Offer clients assistance in completing the Broader Needs Assessment:
- Explain purpose of Broader Needs Assessment to clients.
- Refer individuals to specific programs and/or agencies for service.
- Solicit feedback from clients regarding resources.
- Contact resources to provide client feedback and learn about what they have to offer clients.
- Visit individuals and families in their homes when necessary to explain supportive services and resources available to help them meet their needs.
- Interact with schools, medical personnel, and other agencies to facilitate communication between them and our mutual clients.
- Present information on programs, benefits, and resources to neighborhood groups, schools, healthcare providers, and non-profit organizations.
- Support employees communication with Hennepin County services, client responsibilities, and cultural assimilation techniques.
- Assist in completing forms for program eligibility related issues, gathering verifications to assist in program eligibility determinations, and connecting individuals to health and social service resources.
- Provide education on healthcare systems and the use of public and privately funded healthcare options.
- Maintain accurate records of client interactions, profiles, etc.; prepare reports as needed.
A valid driver's license, access to reliable transportation, and the ability to obtain a Hennepin County driver’s permit for travel to community service providers and home visits throughout Hennepin County.
The ability to communicate fluently orally and in writing in both Dari and Pashto in addition to English.
Candidates with fluency in another language in addition to English are encouraged to apply. Bilingual candidates who pass a language assessment may qualify for additional bilingual pay and duties.
