What are the responsibilities and job description for the RN Cancer Navigator (1.0 FTE) - Breast Cancer position at Hennepin Healthcare?

SUMMARY
Serves as a single point of contact for referring physicians, patients, and caregivers to provide resources and assistance with accessing clinical and supportive care services offered within Hennepin Healthcare and in the community. Collaborates with the healthcare team to coordinate the care of patients across the continuum of care.
RESPONSIBILITIES
- Assist patients in understanding their diagnosis, treatment options, and the resources available, including educating eligible patients about appropriate clinical research studies and technologies. Serve as lead communicator and care coordinator to breast cancer patients
- Serve as an essential link between patients and all other care providers
- Work in clinic with breast surgeons and oncology teams to coordinate care and conduct pre-visit planning and post-visit follow-up to ensure care is appropriate, timely and coordinated for each patient
- Work closely with physicians to allow for efficient and evidenced based care. In these collaborations, build content expertise to allow for clinical decision making
- Facilitate appointments for consults and support services within established service standards, including social workers, dietitians, physician, etc
- Coordinate multidisciplinary planning conferences, develop concise patient summaries for use by the care team, and document recommendations made utilizing standardized care protocols in accordance with nationally recognized care guidelines
- Develop patient education programs and tools
- Follow patients throughout the care continuum. Connect inpatient and outpatient care plans and ensure that care is not lapsed or delayed with transitions between inpatient and outpatient care
- Work with Cancer Registry staff and others to collect data, track outcomes, and support strategic planning processes. Participates in performance improvement activities
- Adhere to established productivity benchmarks
- Responsible for outreach efforts to establish and maintain positive working relationships with key stakeholders (physicians, office staff, diagnostic staff, nurses, radiology staff, social services staff, and radiation oncology staff, etc.)
- Communicates with referring health care providers regarding comprehensive plan of care for patient and family
- Participates in ongoing educational activities to expand clinical knowledge and enhance role performance
- Assist patients with conditions or disparities affecting access to care in achieving quality cancer treatment and follow-up. Work to keep such patients connected to the care team and prevent loss to follow-up
QUALIFICATIONS
Minimum Qualifications:
- Graduation from an approved school of nursing
-AND-
- Three (3) years of professional nursing experience in Breast Cancer Care specialties
- Bachelor degree in nursing or enrollment in a bachelor degree program with progress to complete within two years of hire date
- Breast Care Nurse Certification or able to obtain certification within 2 years of hire date
Preferred Qualifications:
- Requires problem solving, decision making and critical thinking
- Requires excellent leadership, organizational, written, and verbal communication and excellent interpersonal skills
- Must be able to work in a self-directed environment, with an ability to work with and lead teams
- Excellent presentation skills
- Ability to implement professional and community-based education programs
- One year of experience at Hennepin County Medical Center strongly preferred
Knowledge/ Skills/ Abilities:
- Basic computer skills necessary; experience with an Electronic Health Record necessary – EPIC preferred
License/Certifications:
- Possession of a valid license as a Registered Nurse, issued by the State of Minnesota
