Job Summary:
The MDS Nurse is responsible for the accurate and timely completion of the Minimum Data Set (MDS) assessments in accordance with federal and state regulations. This role requires strong clinical assessment skills, knowledge of Medicare/Medicaid regulations, and excellent communication abilities.
Responsibilities:
- Complete accurate and timely MDS assessments, including initial, quarterly, annual, significant change, and discharge assessments.
- Coordinate and collaborate with the interdisciplinary team (IDT) to develop and implement individualized care plans.
- Ensure compliance with all federal and state regulations related to MDS completion and submission.
- Conduct resident interviews and observations to gather information for accurate assessment completion.
- Maintain accurate and organized MDS documentation.
- Educate staff on MDS requirements and updates.
- Participate in care plan meetings and provide clinical expertise.
- Monitor and track Quality Measures and identify areas for improvement.
- Other duties as assigned.
Qualifications:
- Current and valid RN license in Ohio.
- Minimum of 1yr experience working in an MDS Coordinator role required.
- Minimum of 2 years experience as an RN in a skilled nursing facility preferred.
- Experience with MDS 3.0
- Strong knowledge of Medicare/Medicaid regulations and reimbursement.
- Excellent clinical assessment and documentation skills.
- Ability to work effectively within an interdisciplinary team.
- Strong communication and interpersonal skills.
- Proficiency in computer skills and MDS software.
