- Duties will include: caring for patients in the outpatient clinic and hospital settings
- Participating in an after-hours/weekend transplant call rotation is required
- Some transplant experience is helpful
- Communicates effectively with physicians, hospital staff, director, and other healthcare disciplines to facilitate prompt and accurate patient care.
- Analyzes pertinent data, communicates findings with surgeon and/or other physicians, documents information and assumes accountability for decisions and actions of a hospitalized patient.
- Coordinates and facilitates the pre and peri operative processes upon notification of organ availability for a specific patient under the direction of the supervising surgeon.
- Treats hospitalized patient as supported by licensure under the direction of surgeon/physician and transplant center guidelines and within own competency.
- Assists in the care coordination of the acutely ill kidney transplant patient to facilitate communication and treatment with the surgeon and other consultants as needed.
- Plans follow-up care in collaboration with the attending physician, surgeon, and other healthcare disciplines as required.
- Educates patient and family regarding immediate and long term needs related to the post kidney transplant population.
