What are the responsibilities and job description for the Home Care/Hospice Physical Therapist position at Henry Community Health?
Responsibilities
- Assesses the physical therapy needs of patients and families in the home environment.
- Develops plan of care for the patient in conjunction with the physician and other health care providers.
- Administers prescribed care according to physician orders.
- Educates patient family/caregivers on plan of care and provides support to the patient and family/caregivers.
- Works with support services in obtaining needed supplies, equipment and services for the patient.
Qualifications
- Requires specialized knowledge as to administration of treatments and maintenance of equipment.
- Must exercise judgment in administering and analyzing treatments.
- Education requirements are a Bachelor of Science degree and a diploma from AMA approved and accredited school of physical therapy.
- Successful completion of state board examination required.
