What are the responsibilities and job description for the Help Me Grow Home Visitor position at HENRY COUNTY HEALTH DEPARTMENT?
Henry County Health Department’s Help Me Grow Program is seeking to fill 1 part-time Help Me Grow Home Visitor position to provide services in Northwest Ohio. Candidates with previous home-based experience are preferred.
Qualifications/Licensure Requirements:
Minimum of associate degree in social or behavioral science, early childhood development/education, or similar field required, bachelor’s degree preferred; minimum one (1) year experience in home-based therapy, mental health, or crisis intervention service setting; or an equivalent combination of training, education, and experience; Spanish language skills preferred.
Home Visitor credential must be obtained within two (2) months of hire through Ohio Department of Health; if required, must obtain certification as a lactation counselor or child passenger safety technician within 18 months of hire; must maintain a valid State of Ohio driver’s license and maintain insurability according to Health District policy. If the employee holds professional licensure or credentialing (e.g., Licensed Social Worker, Registered Nurse, Infant Mental Health, etc.), license/credential is maintained as required under the Ohio Revised Code.
Responsibilities include:
- Contact families, schedule and complete home/telehealth visits, conduct screenings, documentation, and develop Family Plans.
- Deliver education using required curriculum in English/Spanish, comply with HMG rules and HFA accreditation, coordinate with referral sources.
- Enter HMG program data into computer programs.
- Coordinate with Job and Family Services, provide parental education, attend meetings, liaise with Child Protective Services.
- Provide education and support during events, classes, in-office, and in-home settings.
- Attend meetings, conferences, seminars, maintain required licensure and certification.
- Maintain confidentiality as per Health District policies and laws.
- Demonstrate regular and predictable attendance.
- Meet job safety and OSHA standards.
- Participate in quality improvement teams and initiatives.
- Execute the Health District mission and adhere to its values.
- Complete performance management duties and goals.
- Contribute to achieving the Strategic Plan objectives and goals.

The position(s) will be part- time, 21-28 hours per week. This position is in Pay Grade 4; pay starting at $20.10/hour. THIS IS NOT A REMOTE POSITION.
