Contingent- Day Shift- 8:00am-4:30 pm- minimum of 2 shifts per 6 weeks with 1 shift being on a weekend- no holidays

GENERAL SUMMARY
- Coordinate the overall interdisciplinary plan of care for patient, from admission to discharge.
- Create plan for care across the continuum, integrating patient/family preferences and values.
- Monitor patient care through assessments, evaluations and/or patient records.
- Advocate for resources and removal of barriers.
- Maintain ongoing dialog with supervisor and other health providers to ensure effective implementation of health plan.
- Act as a resource for adequate medical record documentation, appropriateness of services as they relate to diagnoses, and treatment options for post-discharge care
EDUCATION/EXPERIENCE REQUIRED - Diploma from an accredited school/college of nursing OR Required professional licensure at time of hire. - BSN Preferred CERTIFICATION/LICENCES REQUIRED - Registered Nurse credentialed from the Michigan Board of Nursing obtained prior to hire date or job transfer date required. - BLS Provider preferred. American Heart Association accepted. - Case Manager credentialed from the Commission for Case Manager Certification (CCMC) preferred. - Case Manager credentialed from the American Case Management Association (ACMA) preferred.
