What are the responsibilities and job description for the Dialysis Assistant Northland Park - 40 Hours/Week Day Shift position at Henry Ford Health System?
DEPARTMENT DESCRIPTION:
Northwest Northland Park Home Dialysis is a unit attached to an outpatient dialysis facility . The facility oversees peritoneal and home dialysis patients. Census is 20 PD patients and 2 HHD patients.
Staffed with full-time Nephrologists. In addition, we have 2 full-time Home Nurses and travel pool home dialysis technicians. The unit has multiple support staff members that are shared with the outpatient clinic including front desk team and tech leader staff (machine maintenance) that may assist with operational duties within the clinic.
We case manage 15 pts who are monitored by connected health for Baxter and NxStage systems. Patients are trained to perform dialysis treatments at home within the clinic. PD training takes 7-10 days and HHD training takes 4-6 weeks. The home team schedules monthly clinic visits for all home patients with the interdisciplinary team and lab draws for PD patients. Our team provides pt education as well as referrals to transplant services for sites the pt requests to be referred to.
The unit is typically open Monday through Friday 8am to 430pm for pt care hours with some flexibility based on the needs of the clinic. Home Nurses are scheduled to take daily after hour on-call including weekends. The home clinic is closed most weekends and holidays depending on the needs of the clinic (training or clinical need for a patient).
GENERAL SUMMARY:
- Responsible for performing stocking and custodial duties at an assigned dialysis unit.
- Responsible for assisting with implementation of Lean strategies.
- Reports to the Dialysis Nurse Manager and or Technical Leader of the dialysis program, or the charge nurse in his/her absence.
EDUCATION/EXPERIENCE REQUIRED:
