What are the responsibilities and job description for the RN-Patient Navigator-Full-Time position at Henry Ford Health System?
Department: Quality Management Schedule: 8-hour day shift, Monday-Friday Location: Metro-Detroit, MI
- Serve as a patient navigator throughout the continuum of care.
- Function as a liaison for management staff, administration, physicians, managed care companies, community organizations, and other customers.
- Coordinate efforts for treatment and promotes quality improvement and educational efforts.
- Compile data, tracks outcomes, and make recommendations for process improvement.
- Provide referrals/support for follow-up care for patients who are uninsured or underinsured.
EDUCATION/EXPERIENCE REQUIRED
- Diploma from an accredited school/college of nursing OR Required professional licensure at time of hire
- 2 years RN med-surg experience preferred
- Experience with patient care plans preferred
- Experience with Microsoft office suite applications preferred
CERTIFICATION/LICENCES REQUIRED
Registered Nurse credentialed from the Michigan Board of Nursing obtained prior to hire date or job transfer date required .
BLS Provider credentialed from the American Heart Association (AHA ) or American Red Cross preferred
