CONTINGENT weeknights w/ rotating weekends & holidays; Rotating coverage at 3 campuses- Behavioral Health in the Emergency Department; Shift starts at 7:00pm- the hiring leader is flexible with 8- or 12-hour shifts.
GENERAL SUMMARY:
Coordinates the overall interdisciplinary plan of care for patient, from admission to discharge.
Responsibilities:
- Creates plan for care across the continuum, integrating patient/family preferences and values.
- Serves as a resource to assess, evaluate, recommend and treat legal, neglect, substance abuse, crisis, psychosocial and various adjustment needs.| Advocates for resources and removal of barriers.
- Maintains ongoing dialog with supervisor and care transition team members to ensure effective implementation and reevaluation of health plan.
- Acts as a resource for adequate medical record documentation, complex level of care recommendations, and services as they relate to diagnoses, and treatment options for post-discharge care.
EDUCATION/EXPERIENCE REQUIRED
- Master's Degree in Social Work required.
- C urrent licensure by the Michigan State Social Worker required
- Previous Case Management experience preferred
- LLMSW or LMSW or LPC with 2 years of inpatient or ED Behavioral Health Experience preferred
