GENERAL SUMMARY
Responsible for the provision of social work service to renal patients and their families. Will strive to fulfill the organizations mission, values, philosophy and goals.
PRINCIPAL DUTIES AND RESPONSIBILITIES
- Conducts initial psychosocial interviews with patients and families in order to assess baseline level of functioning, comprehension of illness and its ramifications, and impact of individual/family history upon current and future psychosocial functioning.
- Provides ego-supportive counseling to patients and families in order to maximize ongoing psychosocial functioning.
- Provides crisis intervention to patients and families as needed.
- Functions as a patient advocate and as a liaison between the patient and multiple care delivery systems.
- Assists patients with linkage to hospital and community resources based on needs assessment.
- Provides information and referral services as needed.
- Records psychosocial assessments, update assessments, and progress notes in accordance with established recording guidelines.
- Participates in comprehensive inter-disciplinary team (IDT) patient care meetings with appropriate documentation on IDT assessment and plan of care.
- Participates in unit quality improvement meetings and activities.
- Participates in professional development activities, such as attending social work meetings, relevant conferences, professional memberships and professional reading.
- Demonstrates professional judgment in assessing unit dynamics and responds in a manner that will contribute to smooth functioning of the unit; knows when to confront or be supportive.the provision of social work service to renal patients and their families.Demonstrates the ability to communicate effectively to staff, management and/or medical personnel.
- Reports to Senior Social Worker/Social Work Manager.
- Communicates with the multi-disciplinary team members in order to provide quality care to the renal population.
- Works closely with other unit social workers to facilitate patient transfer and information flow between units in order to develop appropriate patient care plans.
- Provide necessary information to the social work technician in order to efficiently and effectively provide concrete services to the renal population.
Qualifications - External
EDUCATION AND EXPERIENCE
- Masters of Social Work degree required, ACSW and previous experience in health care setting preferred. Licensed/Certificate to practice in the State where applicable.
