What are the responsibilities and job description for the Patient Navigator position at Herald Christian Health Center?
I. Job Summary
The Patient Navigator is a person with direct knowledge of the local community who functions as a “guide” and offers assistance to patients in navigating the healthcare system and accessing social services to improve health outcomes. The Patient Navigator performs clerical and medical support services within the clinic setting and in the community. The Patient Navigator will assist patients and family members by identifying their needs and linking them to resources.
This position is grant-funded position and subjects to immediate termination if the grant funding ceases. If the position remains viable, in whole or in part, despite the loss of grant funds, Herald Christian Health Center, in its discretion, may reduce the position to less than full time. The decision to terminate or reduce a position is not grievable.
II. Essential Functions
1. Reduce barriers that keep patients from getting treatment and social services support by identifying patient needs and connecting them to resources (i.e. financial assistance, food access, transportation, etc.).
2. Act as a resource for questions and referrals based on knowledge of health care structure and organizational policies in Los Angeles County
3. Meet with patients to stress the importance of consistent treatment and follow-up.
4. Assist patients with scheduling and completing medical and dental preventive health screenings
5. Coordinate with other HCHC departments for patient referrals
6. Document services provided for each patient and track completed referrals and referrals that are still outstanding.
7. Participate in community outreach events to engage new potential patients.
8. Provide patient updates to Program Supervisor and Program Manager.
9. Provide translation assistance for patients when needed. Demonstrate ability to interpret and effectively explain clinical and technical information both verbally and in writing to patients in plain terms.
10. Attend staff/department and external agency meetings.
11. Participate in staff training as arranged by the corporation.
12. Perform additional duties as assigned.

IV. Education/License/Certification:
1. High school diploma or GED required. Bachelor’s degree preferred.
V. Skills and Specifications:
1. Fluency in a second language (Spanish, Mandarin, Cantonese, or Vietnamese) is required.
2. Previous experience in a healthcare or social services setting preferred.
3. Ability to speak, write, and read English fluently.
4. Ability to work efficiently when faced with frequent interruptions.
5. Knowledge of community resources available in Los Angeles County.
6. Ability to work collaboratively with staff and patients from diverse socio-economic, ethnic, and cultural backgrounds.
7. Communicate effectively orally and in writing.
8. Committed to ongoing learning that supports individual development and community capacitybuilding.
9. Demonstrate maturity, good interpersonal skills, integrity, and professionalism when working with patients and team members.
10. Ability to recognize and deal appropriately with sensitive and confidential issues.
11. Maintain strict adherence to HIPAA guidelines and confidentiality with patient information.
12. Ability and willingness to work some evening and weekend hours.
13. Ability and willingness to travel to different work sites.
14. Proficiency with Google Suite and Microsoft Office.
VI. Physical Requirement
1. Physical effort which may include occasional lifting, bending, stooping or squatting. Considerable walking may be involved. The ability to sit or stand for extended periods of time is required.

Pay: $22.00 - $26.00 per hour
