Job Summary
The Licensed Practical Nurse (LPN) is responsible for providing direct, safe, and compassionate care to residents in a safe manner in accordance with the resident’s care plan and accurately recorded care given. This includes providing and teaching medical diagnosis, medications uses and side effects, and performing nursing procedures, assessing, and reporting resident change to provider.
Expectations
- Maintain a record of regular and predictable attendance.
- Provide nursing care to residents as directed by the interdisciplinary Care Plan Team using safe techniques that comply with the Upjohn Community Care Center policies and regulatory guidelines.
- Assist residents in achieving and maintaining their highest level of functioning in accordance with the care plan and provide input to the Nursing Supervisor for appropriate care plan changes.
- Collaborate effectively with the Interdisciplinary Care Plan Team and families to develop, implement, and review plans of care. Maintains the resident record utilizing the electronic medical record.
- Adhere to acceptable infection control practices including Universal Precautions while providing care to residents.
- Ensure proper care for medical equipment and supplies used for residents to ensure proper care and charges are made
- Accurately document care provided in residents medical record to reflect care rendered and reports significant changes in resident condition to the provider upon completion of the change of condition assessment or focused assessment using the call guidelines provided by Borgess Extendicare.
- Attend training programs and in-services to comply with Heritage Community of Kalamazoo and Upjohn standards and regulatory guidelines, including but not limited to annual skills fair and skills check.
- Maintain a positive attitude and contribute to interdepartmental team by completing assignments in a timely, accurate and professional manner.
- Complies with HCK Customer Service Standards and Upjohn’s- CNA Standards of Care.
- Always Follow Resident Rights.
- Assist new employees in following established Heritage Community of Kalamazoo and regulatory policies and procedures.
- Report any inoperable, faulty, or unsafe conditions regarding equipment, furniture or facilities to the Nursing Supervisor.
- In the event of a facility emergency or disaster, report to assigned areas as directed by the supervisor and perform necessary tasks to ensure the safe handling of residents, staff, and visitors.
- Perform other duties as assigned by the supervisor.
Qualifications
Education:
- Completion of an accredited Licensed Practical Nurse program.
Experience:
- Previous experience in long-term care or a related field is preferred.
Skills:
- Proficient in reading, writing, and comprehending English.
- Ability to follow both written and oral instructions accurately.
- Strong psychosocial skills to interact effectively with residents, staff, families, and visitors.
Certifications/Licenses:
- Must be a Licensed Practical Nurse (LPN) licensed in the state of Michigan.
- Unencumbered by any licensing or state violations.
- Must maintain the required Continuing Education Unites (CEUs).
