We provide comprehensive training and support to help you become a skilled Dialysis Social Worker while delivering exceptional patient care, committed to safety, infection control and well-being!
Work Arrangement: Hybrid remote and on-site, Monday - Friday, No Weekends - Scheduled holidays and overtime based on clinics' preference.
Hours: Flexible hours - Approximate based on the different nursing homes' schedules and physician rounding times.
Location: Greater Saginaw and Bay City areas
Key Responsibilities:
- Executes and monitors the local social service programs and activities in accordance with Company policy, procedures and regulatory compliance.
- Initiates Individualized counseling for patients or families at risk. Conducts initial psychosocial assessment for patients receiving care within the unit.
- Develop treatment goals for initial plan of care and other scheduled assessments per regulatory requirements.
- Assess and Act as advocate on behalf of patient and family.
- Collaborate with our healthcare team, including nephrologists, dietitians, and registered nurses, to develop and implement individualized care plans.
- Provide ongoing written assessments and monitoring of goals.
- Maintain accurate patient records and ensure compliance with regulatory requirements.
- Uphold our commitment to patient safety and infection control protocols.

We are a company comprised of dialysis professionals who specialize in providing Home Hemodialysis in a wide array of settings including but not limited to Skilled Nursing Facilities, Assisted Living, Senior Living, and patients' homes. We also specialize in treating patients with higher acuity needs. We can treat patients who require ventilator care, tracheostomy care, bedside dialysis, special cardiac conditions, and most other high acuity needs.

Heritage Dialysis was started by a Dialysis Nurse who wanted to give people suffering from Kidney Failure better access to dialysis options that would allow them to take back control of their lives. Most people suffering from Kidney Failure are prescribed a standard treatment plan that has them go to an outpatient dialysis clinic three times a week for 3.5 – 4 hours at a time. They are given a time slot for when to show up to the clinic and told what foods not to eat and to limit their fluid intake. All of these things put up a barrier between the life they are used to and the new reality they face with Kidney Failure.

Must Haves
Master's Degree from a graduate School of Social Work, accredited by the council on Social Work Education and LMSW License and/or certification as required by CMS.
Current LMSW licensure in MI.
Responsible for self-professional development and maintenance of necessary credentials.
Excellent communication and interpersonal skills.
A strong desire to specialize in nephrology and provide patient-centered dialysis care.
A strong commitment to patient safety and well-being.
