What are the responsibilities and job description for the Patient Care Liaison position at Heritage Hall Healthcare & Rehab Centers?
Heritage Hall is now recruiting for a Patient Care Liaison. The Patient Care Liaison provides support for the admissions and nursing personnel. Some of their duties include, but are not limited to, new admissions, readmissions, and improving quality control measures. Other job duties may include cleaning and organizing rooms, assisting with transport and tray delivery during mealtimes, and taking care of residents personal items. The Patient Care Liaison may also perform other non-nursing duties as assigned to them by their supervisor.

Patient Care Liaison Qualifications
- High school diploma or equivalent required
- Implement Customer Service Program
- Act as an internal liaison & customer service resource for family members, residents, and visitors
- Schedule resident transports
- Respond to calls in a timely manner
- Knowledge of medical terminology preferred
- Strong written and oral communication skills
- Computer knowledge & data entry skills
