About the role:
Behavioral healthcare navigators have specialized training and knowledge in medical, mental health and substance use disorder resources. They are often more adept at finding appropriate mental health services, support groups, and treatment programs tailored to individual needs. They provide targeted assistance for patients dealing specifically with mental health issues, addiction and/or co-occurring medical problems, which can include crisis intervention, and support in accessing behavioral health and/or medical services. Serves as clinic outreach coordinator resource to identify and meet community needs and advocates for underserved individuals. Implements

The Minimum Qualifications:
• Bachelor’s degree in Human Services field required.
• Must have a minimum of three years of experience in behavioral health setting.
• Medical certification preferred (i.e. MA, CNA, LPN, RN)
• Trained in Mental Health First Aid.
Your Essential Duties:
• Participates as directed in the screening of new patient requests to ensure that all individuals are adequately and appropriately served according to their individual needs.
• Assists with data collection and tracking to support client recovery and identifies treatment deficits.
• Provides ongoing supportive and/or care management functions in accordance with the problems, needs and strategies identified in order to help the clients achieve their goals and objectives.
• Works collaboratively with patients to access services from Heritage Health and outside entities, to include but not limited to 988, Crisis Center, and Mobile Crisis, facilitating effective transitions of care.
• Provide ongoing care coordination and follow-up until they are connected.
• Provide education on co-occurring or co-morbid conditions and treatments, helping patients and their families understand treatment options, and what to expect from the care process.
• Handle mental health crises, providing immediate support and connecting patients with urgent care services.
• Maintains close communication with treatment team for input regarding medication compliance, side-effects and client change of care needs.
• Regular and predictable attendance is an essential function of this position.
• Performs miscellaneous job-related duties as assigned.
Your Success Factors:
• Excellent communication skills with the ability to communicate diplomatically, orally and in writing with both internal and external customers.
• Computer skills – windows-based software with experience in EHR.
• Demonstrates excellent attention to detail.
• Ability to function as self-starter and is goal orientated
• Must be able to simultaneously manage several objectives and assign priorities
• Ability to work effectively under pressure to meet deadlines with a strong organization skill set for easy retrieval of information and to ensure competent closure of issues.
