Key Responsibilities:
- Behavioral Health Assessments: Conduct comprehensive assessments to identify behavioral health/psychosocial needs and develop individualized care plans.
- Leadership and Supervision: Oversee clinical operations and manage ECM team members, ensuring staff compliance with regulatory standards.
- Clinical Direction: Provide strategic leadership for clinical services, guiding clinical managers and interdisciplinary care teams.
- Individual and Family Care: Deliver individual, family, and group therapy sessions, as well as workshops and support groups for members and their families.
- Documentation and Compliance: Maintain accurate and confidential records of all encounters in electronic health records (EHR), adhering to confidentiality standards.
- Community Outreach: Engage in community outreach, providing presentations and participating in health education initiatives.
- Resource Liaison: Serve as a key liaison between members, their families, and external behavioral health resources.
- Care Management and Monitoring: Provide ongoing management of behavioral health conditions, including crisis intervention, counseling, and regular progress evaluations.
- Education: Must have a Bachelor's degree and current licensure (LCSW, LMFT, LPCC, or Psychologist) in the state of California.
- Experience: Minimum 2 years of experience providing direct behavioral health care services.
- Skills: Strong interpersonal, communication, and documentation skills; ability to work effectively with diverse populations.
- Knowledge: In-depth knowledge of behavioral health care practices, Medi-Cal regulations, and various behavioral health interventions (e.g., for Depression, Anxiety, Trauma, SUD).
- Commitment: Deep understanding of health disparities affecting vulnerable populations and a commitment to providing culturally competent care.
- Mobility: Willingness to travel within Los Angeles county for home visits and community engagement.
- Bilingual Skills: Preferred to effectively communicate and serve our diverse member populations.
