Position Overview :
As a Non-Licensed Behavioral Health Care Manager (BHCM) at Heritage Health Network, you will be responsible for addressing the behavioral health needs of our members through the Enhanced Care Management (ECM) program. This role focuses on supporting our diverse member population, ensuring they receive comprehensive, culturally sensitive, and empathetic care, while working closely with interdisciplinary teams and ensuring behavioral health is integrated with other medical and social services.
This role combines remote work, in-office responsibilities, and field engagements, with work arrangements tailored to meet program and member needs.

Key Responsibilities :
Behavioral Health Assessments : Conduct comprehensive assessments to identify behavioral health / psychosocial needs and develop individualized care plans.
Leadership and Supervision : Oversee clinical operations and manage ECM team members, ensuring staff compliance with regulatory standards.
Clinical Direction : Provide strategic leadership for clinical services, guiding clinical managers and interdisciplinary care teams.
Individual and Family Care : Deliver individual, family, and group therapy sessions, as well as workshops and support groups for members and their families.
Documentation and Compliance : Maintain accurate and confidential records of all encounters in electronic health records (EHR), adhering to confidentiality standards.
Community Outreach : Engage in community outreach, providing presentations and participating in health education initiatives.
Resource Liaison : Serve as a key liaison between members, their families, and external behavioral health resources.
Care Management and Monitoring : Provide ongoing management of behavioral health conditions, including crisis intervention, counseling, and regular progress evaluations.
Qualifications :
Education : Must have a Master's degree in Psychology, Social Work, or a related field.
Experience : Minimum 2 years of experience providing direct behavioral health care services.
Skills : Strong interpersonal, communication, and documentation skills; ability to work effectively with diverse populations.
Knowledge : In-depth knowledge of behavioral health care practices, Medi-Cal regulations, and various behavioral health interventions (e.g., for Depression, Anxiety, Trauma, SUD).
Commitment : Deep understanding of health disparities affecting vulnerable populations and a commitment to providing culturally competent care.
Mobility : Willingness to travel within Los Angeles county for home visits and community engagement.
Bilingual Skills : Preferred to effectively communicate and serve our diverse member populations.
