Position Overview :
The Community Health Worker (CHW) at AccessAble is vital in improving community health and enhancing home accessibility through asthma management, home assessments, and environmental modifications. This position requires a compassionate, hands-on professional who understands the challenges faced by individuals with chronic conditions, disabilities, or mobility limitations.
This role is 60%-80% field-based, involving in-home visits to assess living environments, identify asthma triggers, take measurements and photos for home modifications, and educate individuals and families on self-management strategies and accessibility solutions. As a trusted advocate, the CHW bridges the gap between healthcare services and the community, ensuring members receive the resources and modifications they need to live safely and independently.
By fostering strong relationships and delivering direct, in-home support, the CHW helps reduce emergency department visits, improve home safety, and enhance overall well-being. Requirements
Key Responsibilities :
- Perform in-home assessments to evaluate environmental barriers, measure spaces, take photographs, and assess the feasibility of home modifications requested by members.
- Identify asthma triggers in the home and recommend appropriate remediation strategies to improve air quality and reduce asthma-related health risks.
- Provide education and guidance to individuals and families on asthma self-management, accessibility solutions, and home safety improvements.
- Collaborate with healthcare providers, managed care plans, and local organizations to enhance access to home modifications and other support services.
- Document findings from home assessments, compile reports with recommended modifications, and communicate effectively with internal teams and external partners.
- Support the coordination and implementation of environmental accessibility adaptations (EAAs), ensuring members receive necessary home modifications such as ramps, grab bars, and bathroom accessibility improvements.
- Collect and report data on community health needs, home modification outcomes, and the effectiveness of interventions to improve overall health and well-being.
- Stay informed about best practices in asthma remediation, accessibility solutions, and home modifications through ongoing professional development.
