Heritage Home Health and Hospice is growing and currently looking for a Full time Home Health LVN to join our team in Sulphur Springs Tx. To service Sulphur Springs and surrounding areas. Monday - Friday with some call on weekdays and weekend. Pay is competitive. Please apply through really.
Essential Functions:
- Ability to administer skilled nursing care for clients of all ages in their place of residence under the direction of a Registered Nurse.
- Coordinate care with the interdisciplinary team, patient/family and referring agency.
- Participates in planning and implementing care in partnership with the patient, representative (if any), caregiver(s), and the RN. Provides services that are ordered by the physician as indicated in the plan of care.
- Provides patient and caregiver education.
