Responsibilities
Heritage Home Health Licensed Social Workers (SW) work with patients, their families, and other health care professionals in the patients’ place of residency performing visits and completing coordination of care coordination for patients. The SW follows agency state and federal, policies and procedures. The SW works in an interdisciplinary team and coordinates with other agencies in the community to connect care for patients and their families.
Social Workers (SWs) must meet the following requirements:
• Be licensed in the state of Idaho as a Licensed Social Worker (LSW), Licensed Master’s Social Worker (LMSW) or Licensed Clinical Social Worker (LMSW)
